Late cancellation (less than 4 hours before), and no-show fee: $300

Insurance:

All office visits are out-of-network with all insurance companies. I can provide you with a super bill, containing all the codes needed to bill your insurance company for reimbursement. All lab work and diagnostic imaging are billed directly to your insurance company.
